1. Acceptance of Terms

By accessing or using EZColonPrep (“the Platform”), you agree to be bound by these Terms of Use (“Terms”). These Terms apply to all users of the Platform, including patients and authorized healthcare staff. If you do not agree to these Terms, do not use the Platform.

These Terms are between you and EZ Medica, LLC, a Maryland limited liability company (“EZ Medica,” “we,” “us,” or “our”).

2. Description of the Platform

EZColonPrep is a web-based patient education tool that provides personalized colonoscopy preparation guidance, including instructional videos, prep checklists, and optional timed reminders (by text and/or email). The Platform is deployed by medical practices for use by their patients.

You access EZColonPrep because your physician’s practice uses the Platform to support your care. EZ Medica is not your healthcare provider. Your physician and their practice remain responsible for your medical care.

3. Not Medical Advice

The videos, checklists, and reminders provided through EZColonPrep are general educational content. Individual preparation requirements may vary. Your physician’s written instructions take precedence over any content in this Platform.

4. SMS and Email Notification Terms

If you choose to opt into text message (SMS) or email reminders, the following terms apply:

SMS / Text Message Terms

• You authorize EZ Medica, LLC to send you automated text messages related to your upcoming procedure preparation.
• You will receive up to 6 messages over the 7 days before your procedure.
• Message and data rates may apply depending on your mobile carrier and plan.
• Your consent to receive SMS messages is not a condition of receiving medical care or accessing any feature of EZColonPrep.
• To opt out, reply STOP to any message. We will process your opt-out within 24 hours.
• For assistance, reply HELP to any message or contact us at support@ezcolonprep.com.
• The reminder sequence ends automatically on the morning of your procedure and does not continue after that date.

Email Terms

• You authorize EZ Medica, LLC to send you automated email reminders related to your upcoming procedure preparation.
• You may unsubscribe at any time by contacting support@ezcolonprep.com.
• Your consent to receive emails is not a condition of receiving medical care or accessing any feature of EZColonPrep.

5. Your Responsibilities

By using EZColonPrep, you agree to:

• Provide accurate information when entering your procedure details and contact information
• Follow the preparation instructions given to you by your physician, which take precedence over this Platform’s content
• Not share your personalized prep link with others, as it is specific to your procedure
• Contact your doctor’s office if you have any questions about your medical care

6. Intellectual Property

All content on EZColonPrep — including videos, text, graphics, and the platform interface — is the property of EZ Medica, LLC or its content partners and is protected by copyright. You may not reproduce, distribute, or create derivative works from any Platform content without our written permission.

7. Limitation of Liability

EZColonPrep is provided “as is” for educational purposes. EZ Medica, LLC is not liable for any outcome related to your colonoscopy procedure, including inadequate bowel preparation, rescheduling, or any medical complication. Your physician and their practice are responsible for your medical care.

To the maximum extent permitted by law, EZ Medica’s total liability for any claim arising from use of the Platform is limited to the amount paid by you to EZ Medica (which is $0 for patients, as the Platform is provided at no charge to patients).

8. Privacy

Your use of EZColonPrep is also governed by our Privacy Policy, which is incorporated into these Terms by reference. Please review it to understand how we collect, use, and protect your information.

9. Governing Law

These Terms are governed by the laws of the State of Maryland, without regard to conflict of law provisions. Any disputes arising from these Terms or your use of EZColonPrep will be resolved in the courts of Maryland.
